Safe and effective drug usage is the goal of Medication Therapy Management (MTM), which involves collaboration between the pharmacist, the patient (or their caregiver), and other healthcare providers. Its goal is to help people realize the benefits and accomplish the objectives of their prescribed pharmaceutical therapy. As part of Access Care Pharmacy's MTM program, our pharmacists provide services including analysis, consultation, education, training, and monitoring. These offerings are meant to help our clients and patients get the most out of the medications they use. By improving patients' knowledge of pharmaceutical therapy, increasing patients' adherence to prescriptions, reducing healthcare expenditures, and minimizing drug problems, Access Care Pharmacy helps each patient get the most out of their individualized medication regimen. The initial phase of our pharmacist's pharmaceutical treatment management services for a patient is to assess the patient's current and historical medication regimen. Then, our pharmacist will look for any known drug-to-drug interactions, issues, doses, routes of administration, or formulations of the same drug (size, strength, type, etc). As part of this evaluation, we will also examine the patient's current medication schedule in search of opportunities to enhance their health care.
